Office Policy

OFFICE HOURS
We are in the office Monday through Thursday from 8am to 5pm. The office may be closed between noon and 1:30 each day.

EMERGENCIES
If you have a dental emergency, please call the office as soon as possible. Every effort will be made to take care of your dental need the same day. If the emergency occurs after hours, phone our office number for further instructions.

BROKEN APPOINTMENTS
We try to remind by telephone/text/email prior to the appointment, however please do not depend on this courtesy. Your appointment time has been reserved especially for you. If you must change your appointment, we require at least 48 hours notice to avoid a charge. Exceptions to this rule can be determined only on an individual basis according to circumstances. After hours and weekends, please leave a message at our office number.

FINANCIAL ARRANGEMENTS
Payment for treatment is required day of service. Payment of services for the treatment of minors is the responsibility of the adult accompanying the minor. We accept Cash, Check, Visa, MasterCard, American Express, and Discover Card. Returned checks are subject to a $15 charge. Any balance over 90 days will be subject to a 1.5% monthly finance charge.

DENTAL INSURANCE
As a courtesy to you, we will gladly process your dental claim forms as long as we have all necessary information. However, all charges are your responsibility should your insurance carrier deny payment of your claim. Insurance is not an absolute guarantee of payment. Dental insurance plans vary and do not always cover all services. In most instances we will be able to fully explain your dental plan to you. Every effort will be made to ensure that you receive the maximum benefits available under your plan. Your estimated patient share is due day of service. Payment not received from your insurance carrier within 45 days will be due in full and payable by you.

USUAL AND CUSTOMARY RATES
Our fees reflect our commitment to the quality our patients deserve and are considered customary for the area regardless of an insurance company’s determination.

PREVENTATIVE CARE AND RECALL
We believe in preventive care. After your initial check-up and cleaning, you will be advised as to when you will need to return for your exam and cleaning. A postcard will be sent to you as a reminder that you are due. If you did not pre-schedule your six-month appointment, please call our office as soon as you receive your post card to reserve a time for you.
